30pm) SOS Meals on Wheels is a 501-C (3) non-profit committed to providing home delivered meals and a daily check in for vulnerable seniors in our community.
SOS Meals on Wheels produces and delivers 1,600 meals daily for seniors living in Castro Valley, Hayward, Oakland San Leandro, and San Lorenzo.
As advocates for senior citizens, our mission is to provide a nutritious meal delivered by a friendly face that also acts as a wellness check, allowing seniors to remain independent in their homes for as long as safely possible.
Accountability:
The Client Services Coordinator reports to the Client Services Supervisor and is involved with every step of the client intake and assessment process collaborating with the Client Services team members to comply with program requirements and ensure quality of care for SOS Meals on Wheels clients.
Essential Functions and
Responsibilities:
Work with Client Services Supervisor to develop and implement best practices for client intake and assessment procedures.
Partner with Client Services Supervisor and Client Services team to standardize all department recordkeeping practices.
Interview new clients to complete intake assessment and determine their need for service.
Schedule assessments appointments by phone and conduct in-home visits to verify continued status for meals.
Stay current with local senior resources to make appropriate referrals for clients.

Act as one of Agency's main points of contact for partner agencies, APS, and Alameda County.
Update client assessments in SOS database.
Review driver assessments sheets.
Take any appropriate action needed and file.
Assist with nursing student interns as needed.
Position Proficiency Requirements Excellent customer service skills while working with high need populations.
Good verbal and written communication skills The ability to remain calm under pressure in a fast-paced environment.
Highly organized and detail oriented with data and complex paperwork.
The ability to prioritize and balance conflicting demands.
Vehicle in good working condition, a valid California driver's license and proof of auto insurance Basic skills and experience with Microsoft software products (Word, Excel, Access, Power Point).

Effective team building and conflict resolution skills.
Willing to work both independently and as an effective team member.
Preferred education Associates degree in human services or a minimum of 2 years' experience in social services, business administration or related field.
Preference will be given to those whose experience includes similar position working with seniors/disabled persons and/or within the nonprofit sector.
Work Environment Work will mainly be conducted in an office environment.
Job is performed seated or standing for prolonged periods of time.
Regular use of a computer and common office equipment.
Some light lifting and bending required.
Working Conditions:
While in the office the noise level in the work environment can be from moderate to loud.
The work environment will be ever-changing, with staff and volunteers all sharing the same work area.
Driving personal vehicle for in-home assessments.
Spending time in client's homes during assessment.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
